The Facility Locator ServiceThis service has been created to help consumers locate the most appropriate facilities to provide care for their loved ones. Information on this system comes from the state-run licensing database, and facility representatives can add details about amenities and other services to help individuals make informed care decisions.

Facility Types
ALFAssisted Living Facility. ALFs are licensed facilities that provide housing, meals, and personal care services to older persons and disabled adults who are unable to live independently.

AFCHAdult Family Care Home. AFCHs are small, family-type living arrangements, in a private home, to no more than 5 residents. AFCH owners must be licensed, and must reside in the home with the residents.

License Types
Standard LicenseAn ALF licensed to provide housing, meals, and one or more personal care services for a period exceeding 24 hours. Personal care services includes direct physical assistance with or supervision of a residents activities of daily living and the self-administration of medication and similar services. The facility may employ or contract with a licensed nurse to administer medications.

LNSLimited Nursing Services License. LNS is a "speciality" license, obtained in addition to the standard license. It allows an ALF to provide any of the services under a standard license and a limited number of nursing services, such as care of routine dressings, care of casts, splints, etc. Facilities licensed to provide LNS must employ or contract with a nurse(s) who must be available to provide such services as needed by residents.

LMHLimited Mental Health. LMH is a "speciality" license, required in addition to the standard license for an ALF serving three or more mental health residents.

ECCExtended Congregate Care License. ECC is a "speciality" license, obtained in addition to the standard license. It allows an ALF to provide any of the services under a standard license and LNS license including any nursing services permitted within the scope of the nurse's license consistent with ALF residency requirements and the facility's written policies and procedures. A facility with this type of license enables residents to age in place in a residential environment despite mental or physical limitations that might otherwise disqualify them from residency under a standard or LNS license.

Subsidies
OSSOptional State Supplementation is a type of public subsidy for persons who qualify to provide an additional monthly income for room and board in an ALF or AFCH.

ALE WaiverAssisted Living for the Elderly Medicaid Waiver. ALE Waiver is a type of public subsidy for persons that qualify and reside in an ALF licensed for extended congregate care services or limited nursing services.

ACSMedicaid Assistive Care Services is a type of public subsidy for persons who qualify that pays the ALF or AFCH a daily rate for providing assistance with activities of daily living, medication assistance, and health support.

Miscellaneous
ADLActivities of Daily Living include bathing, dressing, grooming, toileting, and ambulating (moving about within or outside the facility).
